Requests for Disclosure, etc. of Retained Personal Information

Disclosure request method

If you would like to request disclosure of personal information held by the University, please consult with the relevant office in advance and submit the "Request for Disclosure of Retained Personal Information" and present or submit documents proving your identity. please.Please note that disclosure requests may be submitted by mail, but disclosure requests cannot be made by telephone, fax, or email.

Disclosure decision

Disclosure decisions, etc. will be made within 30 days from the date of the disclosure request, except in special cases, and will be notified to the disclosure requester.

Request for disclosure method

The person who receives the disclosure should request the method of disclosure by submitting a “Request for Disclosure of Retained Personal Information, etc.” within 30 days from the date of notification of the decision to disclose (including partial disclosure).However, if you wish to make a copy of the information, please attach stamps for the postage and make a request.

Disclosure practice

If you wish to have the information disclosed at the University, please bring a decision notice to the effect that you want to disclose the retained personal information (or a notice of decision to partially disclose the retained personal information) and come to the counter in charge.

Fees

• The disclosure request fee is 1 yen per retained personal information related to the disclosure request.
